For the 2014/2015 school year we are
pleased to announce a new option for submitting an application
requesting a different school assignment. The forms listed below
be completed electronically and sent to student support via email. (You
may also print the document and fax to central
office, but we encourage everyone to use the email version.)
one application per student and complete the application in full, do not
leave any blanks!
1. Click on the name of the file and download to your computer.
2. Complete the form and save using the student’s name.
3. Email the attachment to Student Support Services.
