Athletic Pre-Participation Physical
TO ALL PARENTS: THIS SCREENING IS A LIMITED MEDICAL CHECKUP TO SCREEN YOUR CHILD TO SEE IF
HE/SHE CAN SAFELY PARTICIPATE IN SPORTS, IT IS NOT A COMPREHENSIVE MEDICAL EXAM. IF YOU HAVE
CONCERNS REGARDING YOUR CHILD’S HEALTH, YOU SHOULD SCHEDULE A VISIT WITH HIS/HER PHYSICIAN.
ALSO, THE HEALTH DEPARTMENT IS OFFERING THE TDAP VACCINATION TO ATHLETES AT THIS SCREENING.
PARENTS MUST SIGN THE APPROVAL FOR THE VACCINATION ON THE NEW PHYSICAL FORM.
The boys’ Athletic Pre-Participation Screening Exams for the 2008-2009 school year are scheduled
for Thursday, June 12, 2008. The New Hanover County Health Department at 2029 S. 17th Street
will be the site for the screenings this year. Schools will be admitted in the following order:
6:00 P.M. Pop Warner Teams
6:30 P.M
7:00 P.M
Roland Grise M.S.
Myrtle Grove M.S.
New Hanover H.S.
Virgo M.S.
Williston M.S.
Hoggard H.S.
The following procedures must be followed so physicians can administer exams as orderly as possible:
1. Athletes must use the NEW TWO PAGE Athletic Pre-Participation form adopted by the New Hanover County School System. Forms are available at all middle and high schools and the Board of Education Office or by going to the New Hanover County Schools web site http://www.nhcs.net/forms/athletic.pdf. If you have the old four page outdated physical forms, please discard them.
2. A parent or guardian must complete the permission, emergency information and medical history parts of the exam and sign where indicated at the end of the form before the exam. The Health Department is offering the Tdap vaccine as a part of the screening only if parents initial on the form where indicated.
3. ATHLETES DO NOT NEED TO ARRIVE EARLY TO GET IN LINE. SCHOOLS ARE ADMITTED IN THE ORDER LISTED ABOVE. IT IS NOT FIRST COME, FIRST SERVED.
4. Athletes must wait in the parking lot at the side of the building.
5. This is the only date for free screenings.
6. Rising ninth graders should list the high school they will be attending next fall.
ATHLETES AND PARENTS PLEASE DO NOT CALL THE HEALTH DEPARTMENT. THIS INTERRUPTS THEIR BUSINESS DAY. ADDITIONAL INFORMATION CAN BE OBTAINED BY CALLING JIM STRICKLAND AT 251-6100X270.
