HOME/HOSPITAL INSTRUCTION
The
New Hanover County Schools system provides Home/Hospital Instruction to
students so that they may continue to succeed even under unusual circumstances.
Home/Hospital instruction is managed through the Exceptional Programs
Department. Instructors are certified
teachers who have a variety of experience and specialties to serve the needs of
ALL students in the system.

& Answers
Who can receive Home/Hospital services?
- Students who are enrolled in New Hanover County Schools and meet the criteria
- Students who have a medically documented condition requiring four or more weeks out of school
- Pregnant school girls who have a pre-natal complication or who have given birth (see Home/Hospital Pregnant Student Referral, Pregnant Student Checklist and Medical Evaluation)
- Medically fragile students
- Students with an IEP who need intermittent services
How are these services accessed?
- A Request for Intervention is made to a school's Student Support Team
- The Physician's Statement, Psychological Physician’s Statement and parental consent forms are completed and returned to the school
- The Student Support team meets with the parent (see Home/Hospital Notice to Parents) and writes a Plan for delivery of H/H Instruction services
What does the Plan for Intervention typically include?
- Adaptations of assignments if needed
- Conditions of service delivery including attendance requirements, time and location
- Designation of a case manager
- Accommodations at school when student returns
What will the case manager do?
· Facilitate communication between all parties involve
· Coordinate the flow of assignments
· Keep everyone informed of the student's condition
When the student is out for an extended period of time, how will tests and grades be given?
The Hospital/Home teacher will give tests to the student in a timely manner as needed with appropriate accommodations for physical limitations. The class teacher will determine grades with input from the H/H teacher as needed.
Why is an exchange of information needed with the physician?
A school official must re-certify extended time beyond the initial period indicated by the physician. Consultation is often necessary to plan a transition back to school.
Who will the parent or physician contact at the school if they have questions or problems?
The case manager is the appropriate person to contact. This person will be someone who will advocate for the student and notify administrators if there are problems.
Why is an Intervention Plan needed for expectant students before H/H services are needed?
- The case manager can help the student with any issues that might arise through a proactive suggestion list.
- Teachers will be able to encourage the student to continue her education.
- The student will be less likely to have physical complications.
- The student will be more likely to complete her education with support from the school environment.
What can a parent do to help a student who is out of school for an extended period?
- Encourage the student to plan their time to accomplish the assigned work.
- Provide any needed accommodations such as: good lighting, a lap desk, a quiet time to work, help with reading or writing, providing extra materials from libraries or other sources when needed.
- Keep the school notified of medical changes and advice from the doctor.
What can H/H services do for the frequently absent student who has a chronic medical condition?
If the student has a 504
Intervention Plan in place, the plan could include a tutorial visit with
assignments from the school. The time before notification of H/H would be
determined on an individual basis.
