Field
Trip Scheduling Process |

Please
follow this process for scheduling field trips during the school year.
The
Field Trip Request Form 5119 is used when scheduling all
field trips. A copy of the form can be downloaded here by

Field
Trip Request Form
The
step by step process listed below should be followed to improve the
documentation process and ensure that buses are available and have
been reserved:
1.
The
requestor will check availability of buses by
completing the electronic copy of Form 5119, saving it as a Word
document, and E-mailing it to the
Field Trip Coordinator Valeria Jones (vjones@nhcs.net
)
in the Transportation Department at least 2 weeks in advance of the
field trip date.
2.
The
Field Trip Coordinator will review the request and notify the
requestor by E-mail that buses are available or that there are
conflicts with scheduling the field trip. (Note that field trips
during school hours using regular bus drivers may only be scheduled
from
9:00
a.m.
to
1:30
p.m.)
3.
The
requestor should then complete the Field Trip Request Form (see link
above), obtain required approvals and have the Principal or their designee (Asst. Principal)
forward the electronic copy attached to an E-mail stating they approve
the trip to the Field Trip Coordinator within 3
school days after the availability is verified. The field trip will
not be officially scheduled until the approved
copy is received by the Field Trip Coordinator. This approved copy
should also be sent by E-mail to Glenda White (GWHITE@nhcs.k12.nc.us)
in the Instruction and Academic Accountability Department.
4.
If
a field trip has to be cancelled, the requestor should notify the
Field Trip Coordinator by E-mail as soon as possible.
5.
The beginning date for taking all field trips during the school year
is October 1st. However, no field trips may be taken after June 2nd.
